Though the tests we face in an aging population may seem giant, our Always Best Care pledge is to make them manageable.<\/p>\n

\"Family<\/p>\n

Our Family Support Services provide a continuum of resources that help caregivers reach higher levels of competency, success, and emotional well-being. Programs include monthly caregiving trainings, bi-weekly counseling services, and intensive care management. As part of our mission to improve quality of life for ALL seniors in the community, many of our programs are complimentary or o\ufb00ered on a sliding scale and open to the public.<\/p>\n

Care Management<\/em><\/strong><\/p>\n

Care Managers at Always Best Care have only one job\u2014to provide out-of-the box solutions, support, and resources for optimal aging.\u00a0 The bottom line: we strive to provide guidance, clarity, and momentum towards reaching care goals and maintaining your independence.<\/p>\n

ABC has two degreed gerontologists on staff and a Certified Senior Advisor. Our Care Management Team is supervised by a Ph.D. level mental health clinician specializing in dementia care.\u00a0 Combined, we have over 50 years of experience serving older adults and their families.<\/p>\n

Most Care Manager visits are included at no additional charge for clients who are receiving regular in-home care services.\u00a0 For families requiring Care Management and do not receive in-home care services through our organization, we have a program for you too at an affordable price.<\/p>\n

Our Care Managers:<\/strong><\/p>\n

    \n
  • Facilitate thorough evaluations of your needs within several domains, including: crisis intervention, health, legal, housing, and community resources.<\/li>\n
  • Develop a care plan that addresses concerns identified during the evaluation<\/li>\n
  • Provide regular home visits to assess and supervise our home health aides.<\/li>\n
  • Attend care conferences with various members of the patient\u2019s medical team.<\/li>\n
  • Arrange medical appointments.<\/li>\n
  • Participate in family meetings to review care options and advocate for the patient.<\/li>\n
  • Provide resources and referrals for additional services and support.<\/li>\n
  • Arrange additional therapies, including psychotherapy and physical and\/or occupational therapies.<\/li>\n
  • Serve as a \u201cproxy\u201d for out-of-town family members.<\/li>\n<\/ul>\n

    Caregiver Counseling<\/em><\/strong><\/p>\n

    Our counseling services offer you support and the opportunity to share feelings in a safe environment.\u00a0 Counseling may help to identify your needs and provide you with the best resources, education, and tools to cope effectively with the challenges of caregiving. Discussion topics may include:<\/p>\n

    • Adjusting to the Diagnosis<\/li>\n
    • Crisis Management<\/li>\n
    • Caregiver Skills-Building<\/li>\n
    • Short-Term Problem Solving<\/li>\n<\/ul>\n

      Caregiving Essentials<\/em><\/strong><\/p>\n

      This complimentary three-hour workshop is open to the entire family and empowers caregivers with the knowledge and coping skills needed to effectively manage the day-to-day challenges of caring for a loved one with memory impairment, Alzheimer\u2019s disease, or a related dementia.<\/p>\n

      Attendees will:<\/strong><\/p>\n

        \n
      • Receive an overview of dementia and Alzheimer\u2019s disease, including the continuum of changes that occur over the course of the disease<\/li>\n
      • Discuss effective strategies to communicate with persons in the early to late stages of dementia and Alzheimer\u2019s disease<\/li>\n
      • Learn adaptable techniques to help manage challenging behaviors that can be present in persons with dementia and Alzheimer\u2019s disease<\/li>\n
      • Process the emotional impact of caregiving and how to cope with challenging behaviors<\/li>\n
      • Develop a plan to reduce stress and take care of yourself<\/li>\n<\/ul>\n

        Caregiving Essentials is generally offered on the 3rd Saturday of the month from 9am-12:30pm and is taught by David Hart, Ph.D. \u2013 a clinical mental health counselor with nearly 15 years of professional experience working with people with dementia and their caregivers.\u00a0 RSVP is required by calling (310) 503-6893.<\/p>\n
