Memory Care
Our Memory Care Team, with David Hart, Ph.D. at the helm, takes pride in our capacity to meet the needs of patients at every stage of the disease process, whether it’s Alzheimer’s disease or a related dementia. We are committed to improving the quality of life for all affected clients and their families and care partners. We achieve our mission through the implementation of proven strategies shown to produce results for some of the more difficult challenges.
Here’s how we provide families with concrete results:
- The Memory Care Team (MCT) provides face-to-face training to our caregivers, utilizing the Memory Care Tools strategy, developed at Hoag Hospital to manage some of the more difficult behaviors frequently exhibited by patients with dementia. Training is on-going and includes crucial clinical information on effective communication strategies to engage our patients with dementia; specific tools to redirect agitated and aggressive behaviors that are often associated with confusion; and comprehensive approaches to developing activities that stimulate the patient cognitively, socially, physically, and spiritually. Most importantly, our caregivers continue to be supervised by the MCT throughout their tenure, receiving on-going continuing education on specific patient needs related to the changing stages of dementia.
- The MCT, in conjunction with the patient’s family, develop individually tailored Memory Care Plans that address the myriad of cognitive, behavioral, and functional needs present at varying stages of the disease process. Memory Care Plans function primarily as road maps that guide the patient, family, caregivers, and MCT to the same destination – ultimately a higher quality of life for the patient with dementia and the family as a whole. Memory Care Plans also assist all parties to maintain an unparalleled level of accountability that is unmatched in the in-home care industry.
- Family members have access to several support services and educational opportunities. Each family is assigned a primary Care Manager who oversees the implementation of the Memory Care Plan as well as supervises the caregiver(s) assigned to the case. The Care Manager is accessible to provide support to the family or address any challenges that may arise over the course of care and frequently visits with the family, caregiver, and patient during the first 3 months of service. Hart is also available for family consultations and one-on-one counseling, particularly for challenges that require his expertise. We recognize that our services may not be affordable for all families. Our Memory Care Team is happy to provide resources and referrals to any community member in need of information and direction.
