Rehab/Home Health teams do great work. We partner with Home Health to help make the transition home safe and complete by supporting routines, ADLs, meal prep, engagement, and confidence while strength returns.

Program snapshot:

How it works:

  • Pre-Discharge Training: shadow therapy team; assess strengths and support needs
  • Assessment at Home: safety inspection, goals, plan of care
  • Start of Care + Training: formal start; 3-day check-in; then every 2 weeks
  • Graduation: reassess, evaluate goals, celebrate independence

Families often want extra support during high-risk hours — especially with confusion, fall risk, or when family can’t be present. We can provide temporary companion/sitter support (as appropriate to the facility setting) to improve safety, comfort, and family peace of mind.

Home care is personalized support delivered in the comfort of your loved one’s own home (help with daily tasks, companionship, personal care, errands, etc.), while assisted living is a senior housing option with 24/7 access to care in a community setting that is great for socialization and engagement. At Always Best Care, we start with a care consultation to understand needs, strengths, challenges, and goals. From there, we help you compare options based on health, safety, independence desired, family support, and budget. We can assist you with both in‑home care and assisted living placement guidance so you can make the best choice for your situation. We can even provide certain types of medical care in the comforts of your home. These include wound and catheter care, feeding tube, etc. that most assisted living communities are unable to assist with.

Consistency in care is very important to us. To achieve that, we prefer starting about one week after your initial consultation and care plan is agreed upon. This helps us make the best match between our clients and Care Assistants. However, we can often begin care very quickly depending on urgency and caregiver availability.

Our specialty is caring for those with neuro cognitive issues like dementia, Alzheimer’s, Parkinson’s, ALS, etc. To ensure quality, we only hire Care Assistants with professional dementia care experience. In lieu of generic dementia and other training, our Care Supervisor trains Care Assistants specifically for each client’s needs, since every individual’s needs are unique. This is key to our success.

Most clients want consistency. And most employees prefer consistent fulltime work. To ensure consistent quality of care for clients and peace of mind for families, our clients typically use us for 30+ hours/week. Shift lengths vary anywhere from 6 to 12 hours, depending on each situation and need. We are sometimes able to assist clients for even 2 hours/day, without compromising quality, but at a much higher cost.

Absolutely. We offer senior living referral services (we generally receive a referral fee from communities for this specialized service). Our local senior living specialists, abiding by the NPRA code of ethics, work one‑on‑one to help identify and visit independent living, assisted living, memory care, family care homes, and other community options that fit your loved one’s caregiving needs, lifestyle preferences, budget, and location.

Your satisfaction is very important to us, and so is the satisfaction of our employees. If the Care Assistant assigned isn’t a good match for your loved one’s personality or needs, we’ll work with you to find a better match. Our goal is a strong, trusting relationship between Care Assistants and client.

Yes. Our team regularly supports families after a hospital or rehabilitation stay — providing care at home during recovery, helping with daily tasks, medication reminders, mobility assistance, and coordinating with other care providers to ensure a smooth transition home and back to independence. Please ask about our Transition to Independence specialty program.

Yes — in addition to home care, we can arrange caregiver support within independent living, assisted living, memory care, skilled nursing, or rehabilitation facilities if extra personal assistance is needed beyond the facility’s staff.
