Absolutely! Federal law gives patients the freedom to choose their home healthcare provider. It would be our honor to care for you, or your loved one when you need us most!

Home health is designed to help you recover in the comfort of your home through skilled nurses and rehabilitation services. Home health must be ordered by a physician when they determine that your condition requires you to stay at home for rehab and recovery from illness, injury or surgery. Home health is the most cost effective option for receiving care as it allows for patients and their families to be home where they want to be.

Home health is skilled clinical care ordered by a health care provider. These services might include things like nursing, physical therapy and occupational therapy, and are designed to help you manage your condition and regain your independence after illness, injury, or surgery.
Personal home care assistance involves non-clinical services, such as help around the house, transportation, bathing, and light meal preparation.

Paying for home health services can be done through various methods, depending on your specific situation and the options available to you. Here are some common payment methods:

Medicare: If you qualify for Medicare, it may cover 100% of your home health services. Medicare is a federal health insurance program primarily for individuals aged 65 and older, but it can also cover younger individuals with certain disabilities. To be eligible for Medicare coverage of home health services, you typically need a doctor’s order and meet specific criteria related to your health condition.

Private Insurance: Many private health insurance plans cover home health services. The coverage and requirements can vary significantly from one insurance plan to another, so it’s essential to check with your insurance provider to understand your specific coverage and any out-of-pocket costs you might incur.

Medicare Advantage: Some individuals choose to enroll in Medicare Advantage plans (Part C), which are offered by private insurance companies approved by Medicare. These plans often include coverage for home health services, but the specifics can vary by plan.

  • IV Therapy: Intravenous therapy at home is a way to receive IV treatment your doctor has prescribed, without having to go to a hospital or clinic. Home IV Therapy is safe and effective alternative to in-patient care.
  • Chronic Disease Management: We help patients and their families manage disease process through care, education, and support.
  • Wound & Ostomy Care: We have Wound Care & Ostomy Specialist who provide the highest level of medical services for hard-to-heal wounds, as well as ostomy care, including colostomy, ileostomy, fistulas, and leaking tube/drain sites.
  • Therapy Services: Our physical, occupational, and speech therapists will come to you or your family member’s home to evaluate your loved one’s health and physical abilities and coordinate a complete therapeutic care plan with your loved one’s doctors to restore his or her best health.

  • Cancer Care: If you or a loved one is recovering from surgery or undergoing chemotherapy or radiation treatments your health care needs may be complex. Therapists and in-home health professionals will come to you to provide the care you need in the comfort of home. You can count on us for:

    • Resource planning for help with meals, transportation, applying for medical assistance, advocating with insurance companies, and support to stay comfortably at home

At your first visit a nurse will conduct an initial comprehensive assessment through a combination of interview and hands on assessment. The assessment will focus on identifying areas where you may benefit from education on self-care management in addition to providing hands on intervention. Your nurse will review all your medications and make sure you are taking them correctly and safely, as well as assist you in setting them up if needed. Your nurse will collaborate with your physician and determine the level of care you need. This approach helps to allow you to stay in the comfort of your home and actively engages you and your caregiver in your health care.

The frequency and type of home health visits and the services provide are based on your individualized plan of care. Your doctor may change your plan of care, increasing or decreasing the number of visits or services provided, in order to provide you with the best care possible.

An individual shall be considered to be “confined to his home” if the individual has a condition, due to an illness or injury, that restricts the ability of the individual to leave his or her home except with the assistance of another individual or the aid of a supportive devise (such as crutches, a cane, a wheelchair, or a walker), or if the individual has a condition such that leaving his or her home is medically contraindicated. While an individual does not have to be bedridden to be considered “confined to his home”, the condition of the individual should be such that there exists a normal inability to leave home or that leaving home requires a considerable and taxing effort by the individual.

Yes. If you or a loved one is interested in receiving skilled home health care services, speak with your primary care physician, nurse practitioner, or other health care provider.

Personal home care is often needed alongside skilled home health services, and we can help coordinate the two levels of care. A limited number of your personal home care hours may be covered by Medicare while you’re receiving clinical in-home care, but it’s generally paid separately or covered by private insurance or other government programs.
